About the Role
We are seeking a compassionate and dedicated Community Worker to join our team. In this role, you will provide essential support and assistance to individuals, families, and groups within the community who are experiencing personal, social, or economic difficulties. You will work collaboratively with clients, community organisations, government agencies, and other stakeholders to identify needs and coordinate access to appropriate services and resources.
This position is classified under ANZSCO ****** — Community Worker and aligns with the Australian and New Zealand Standard Classification of Occupations, as recognised by the Department of Home Affairs (Australia).
Key Responsibilities
●Conduct needs assessments and interviews with clients to identify personal, social, emotional, and practical support requirements
●Develop, implement, and review individual support plans in collaboration with clients and relevant stakeholders
●Provide information, referrals, and advocacy to connect clients with appropriate community services, government programs, and support networks
●Assist clients in accessing housing, financial assistance, healthcare, legal services, and other community resources
●Facilitate individual and group programs, workshops, or activities that promote community participation, wellbeing, and social inclusion
●Maintain accurate, confidential, and timely case notes, progress reports, and client records in accordance with organisational and legislative requirements
●Liaise and build partnerships with government departments, non-government organisations (NGOs), healthcare providers, and community groups
●Monitor and evaluate client progress, adjusting support plans as required
●Support clients experiencing crisis situations, providing appropriate intervention and referral to specialist services
●Participate in community development initiatives and contribute to the planning and delivery of community programs
●Adhere to all relevant legislation, professional standards, codes of ethics, and organisational policies
●Attend team meetings, supervision sessions, and ongoing professional development as required
Selection Criteria
Essential
●Diploma of Community Services (CHC52021), or Bachelor of Social Work / Social Science, or a related qualification at AQF Level 5 or above
●Demonstrated experience working with individuals and communities in a human services, community services, or social support setting
●Sound knowledge of community services systems, government support programs, and relevant legislation (e.g., Privacy Act 1988, relevant state/territory community services legislation)
●Highly developed interpersonal and communication skills, including the ability to engage sensitively with people from diverse cultural backgrounds
●Ability to conduct needs assessments and develop client-centred support plans
●Strong case management, documentation, and report-writing skills
●Ability to work both independently and as part of a multidisciplinary team
●Commitment to professional ethics, confidentiality, and duty of care
●Valid Working With Children Check (or equivalent) and National Police Clearance (or willingness to obtain)
●Current driver's licence (preferred, depending on location)
Desirable
●Experience working with specific population groups such as Aboriginal and Torres Strait Islander peoples, CALD communities, people experiencing homelessness, people with disability, or those affected by family violence
●First Aid Certificate (Level 2 or equivalent)
