ERMA Care Pty Ltd is an approved NDIS provider based in Ashgrove, Queensland. We provide person centred support to people living with disability, older people, families and community members who need practical support to live with dignity, independence and choice.

Our services include community nursing care, personal activities, transport assistance, group and centre based activities, life stage transition support, household tasks, shared living support, accommodation and tenancy support, community participation and innovative community participation.

At ERMA Care, we believe support should not be delivered in a one size fits all way. We work with each participant, their family and their wider support network to understand their goals, barriers, strengths and choices, and to help them participate more actively in community life.

About the role

Due to continued growth, we are seeking a full time Community Worker to join our team in Brisbane. This role will focus on helping participants identify their needs, access community resources, improve independence, connect with services and participate meaningfully in social, educational, recreational, housing, health and employment related activities.

This is a mobile community based role. You may work with participants in their homes, shared living arrangements, community settings, group activities and other service locations across Brisbane and surrounding areas.

Key duties

  1. Assess participants’ needs, goals, strengths, risks and personal circumstances to identify the type of community and welfare support required.
  2. Interview participants and, where appropriate, consult with families, carers and support coordinators to understand the nature and extent of social, emotional, housing, health, recreational, transport, training or employment related barriers.
  3. Develop, review and implement individual support and community participation plans that promote independence, inclusion, safety and wellbeing.
  4. Assist participants to access suitable community resources, including health services, housing support, education, training, employment pathways, recreational activities, advocacy services and welfare agencies.
  5. Liaise with families, carers, NDIS support coordinators, allied health professionals, community organisations, government bodies and other service providers to coordinate appropriate support.
  6. Provide information, guidance and practical support to help participants understand available services, make informed choices and participate in community life.
  7. Monitor participant progress against agreed goals and prepare accurate case notes, progress reports and service records.
  8. Identify changes in participant circumstances and escalate concerns where additional support, referral or review is required.
  9. Refer participants to specialist agencies or external services where their needs are outside ERMA Care’s direct service scope.
  10. Support participants during life stage transitions, including changes in accommodation, daily living arrangements, community access, education, training or social participation.
  11. Promote awareness of community resources and encourage participants to build confidence, social connection and independent living skills.
  12. Work respectfully with people from diverse cultural, social and personal backgrounds, while maintaining confidentiality, dignity and professional boundaries.

Selection criteria

  1. Diploma of Community Services, Diploma of Disability, Diploma of Mental Health, or a related qualification.
  2. Experience working in community services, disability support, welfare support, case coordination or a similar participant focused role.
  3. Strong understanding of person centred practice, participant choice, community inclusion and NDIS service delivery.
  4. Ability to assess client needs and prepare practical support plans, case notes and progress reports.
  5. Strong communication skills and the ability to liaise professionally with participants, families, carers, service providers and community organisations.
  6. Current NDIS Worker Screening Check or ability to obtain one.
  7. Current Working with Children Check or Blue Card, if required for the role.
  8. Current National Police Check.
  9. Current First Aid and CPR certificate.
  10. Valid driver licence, reliable vehicle and willingness to travel between participant locations.
